The principle of equivalence has proved difficult for the Prison Service to achieve. Reed & Lyne (2000) in a number of semi-structured inspections of prisons make a variety of criticisms of the care of prisoners with mental health problems. Prison hospital wings are often poorly staffed with many running problems. For example the doctors often have little psychiatric training and the other healthcare staff also have little specific mental health training (Reed & Lyne, 2000). The amount of therapy being carried out with prisoners is failing to reach standards that have been set - prisoners, on average, are out of their cells only 3.5 hours a day compared to the target of 12 hours in a day. On top of this, for more serious cases, transfers to NHS hospitals are generally slow (Reed & Lyne, 2000).Other than those with mental health problems, a variety of other minority groups are also identified by the HCHAC (2004) as being at greater risk. As women make up only 6.1% of the prison population, they are considered a minority (Home Office, 2003). The majority of these women have children of their own under 16, poor education and occupational opportunities. Many women are in prison after drug-related offences and, unsurprisingly, also have drug problems. The Prison Service has made little specific provision for women, with programmes tending not to address their specific needs. Young prisoners are a further minority group which have not received the amount of rehabilitative hours that have been targeted for them. As the HM Chief Inspector of Prisons (2004) found in the annual report, educational hours have been falling, despite already being at only half their target level. Finally, the HCHAC (2004) report identifies the possibility that ethnic minority prisoners are not receiving the same level of access to constructive activities that other prisoners are gaining.
